2. Accomplished Complete Health Declaration Form that can be found at the hotel.

As part of the Coronavirus (COVID-19) restrictions, the property may request additional documentation from guests to validate identification and other relevant information. 
For unvaccinated / partially vaccinated guests:
1. Negative RT-PCR (swab) test result (DOH-accredited molecular laboratory) or negative Saliva RT-PCR test result, taken within seventy-two (72) hours before check-in.
2. Accomplished Complete Health Declaration Form that can be found at the hotel.
3. 1 Valid ID.
